Day 3
Riverbend's attendance did not post by 6 PM. We saw it before the owner did, checked whether the sync failed or staff had not entered it, retried the sync, and sent the standard “attendance not yet posted” note. Logged.
What it could have cost
Unposted attendance means an unknown last date of attendance. One wrong LDA on one R2T4 is a repayment liability plus a finding — at Riverbend's tuition, a single misdated withdrawal can mean $3,000–$8,000 returned to the Department, and the finding goes in the visit report.
Day 20
A card-on-file charge failed. The standard reminder went out. A person decided what happened next — the system never retries a payment on its own.

Day 60
Agent run #412 timed out. It was retried, succeeded on the third attempt, and the recovery was logged. The owner never heard about it. That is the point.

Run #412 was the nightly LMS-vs-records-vs-money reconciliation. One missed night hides whatever went wrong that day. Found at month-end, a ledger mismatch is hours of staff time. Found by the examiner, it is a finding on financial records.
Day 90

A records finding at an ACCSC visit typically means a written response, 60–90 days of staff time, and a follow-up review. Repeat findings can lead to show-cause. Loss of Title IV eligibility would remove most of a 60-student school's tuition revenue.

Is Riverbend a real school?
No. Riverbend is a composite built from the career schools we work with: 50–80 students, one or two allied health programs, ACCSC or ABHES, a state license, and Title IV. The numbers are real tier pricing; the school is illustrative.
Do I have to start at Tier 03 to get this?
No. Most schools start at Protected and move up. Everything in this example at Tier 01 — locked records, SAP, the Examiner Binder — is available on Day 1 for $10 per student.
